Referrals

A referral from a primary care or other doctor is needed to obtain an appointment.

Effective Sept. 3, 2007, the Center of Hope established new procedures for referrals. Here are the steps doctors need to complete:

Complete in entirety and fax the Center of Hope Referral Form to 210-704-4237, along with medical records relevant to the chief complaint. The referral form provides our clinic important information, including demographics for the patient and primary and/or referring physician and the diagnoses, chief concerns and reason for the referral. Please ensure all items are completed, or the referral form may be returned to your office for completion. Also, ensure the patient has authorization for the visit, if required by the insurance provider.

Once the referral has been received, your office or the family may call Central Scheduling at 210-704-4100 to obtain an appointment date and time.

Prior to the scheduled appointment date, a clinic staff of the Center of Hope will contact the family to complete questionnaires and obtain medical and educational records. This information is essential to a complete evaluation. All preclinic paperwork, including relevant medical and educational records, will be required prior to the appointment. If they are not received, the specialist may reschedule the appointment.
